A wnewch chi lenwi y ffurflen ganiatad os gwelwch yn dda os ydych yn cytuno i’ch plentyn gymryd rhan. Ni fydd modd i’ch plentyn gymryd rhan os na chawn ymateb i’r ffurflen ganiatad.

Please sign the consent form if you are happy for your child to participate. Your child will not be able to participate without written confirmation.
